The Wellness Social Work Assistant/Case Managers are responsible for providing services to WeCARE clients who are deemed to be temporarily unemployable. The Wellness Social Worker Assistant/Case Manager’s role is to ensure that clients receive the support they need to adhere to the Wellness Plan including referrals and assistance to remove barriers to compliance such as transportation. The Wellness Social Work Assistant/Case Manager will also provide outreach to remind clients of pending appointments and attempt to re-engage clients if they have missed an appointment.

Additional duties as needed to ensure compliance with HRA’s and UBA’s expectations.

Qualifications:

  • Bachelor’s degree in human services or related field required. Master’s degree program in human services or related field preferred.
  • Minimum of 1-2 years of experience providing direct services to people with psychosocial barriers to employment. Effective oral/written/interpersonal communication skills required. Bilingual skills preferred but not required. Basic computer skills required.
  • Able to assist clients to access services to treat medical/health diagnoses; Able to assist clients to access services to treat behavioral health diagnoses.
  • Knowledge of various medical software systems including EPIC, PSYCKES, SEAMS
