Your Responsibilities Include:

• Performing work in the citizen’s own home in accordance with the Service Act.
• A rehabilitative approach to citizen care (holistic care).
• Involving the citizen and any relatives in goal-setting.
• Quality assurance and health professional documentation; FS III.
• Collaborating with municipal actors such as nurses, visitation, and the assistive devices center.
• Participating in professional and triage meetings.
• Flexibility and handling complex citizen relationships and situations.
• We operate with a contact person system and fixed driving lists.

We Expect You To Have:

• Education as a Social and Healthcare Helper or Social and Healthcare Assistant.
• Knowledge of Cura and/or Nexus is an advantage but not necessary.
• A driver’s license and a clean criminal record.
