Key Responsibilities:

* Conduct Initial Mental Health Assessments and develop Support Plans
* Understand and manage Delayed Transfer of Care (DTOC) cases
* General Case Management and needs assessments

What We’re Looking For:

* Qualified social worker with 2+ years of experience in adult mental health (required)
* SWE registration (required)
* Professional qualification in Social Work (required)
* Understanding of hospital discharge processes and DTOC (required)
