Job Overview

We are seeking a motivated and experienced Band 6 Occupational Therapist to join the community team at Shropshire Community Health NHS Trust. The role involves delivering patient-centered care in a community setting, supporting rehabilitation and promoting independence.

Responsibilities

* Manage a caseload of patients in the community, assessing their functional needs and developing tailored intervention plans.
* Conduct holistic assessments of patients’ physical, cognitive, and emotional needs.
* Work collaboratively with multidisciplinary teams to deliver integrated care.
* Provide rehabilitation to patients with complex conditions, enabling them to achieve independence in daily activities.
* Deliver equipment recommendations and adaptations to improve patients’ quality of life.
* Offer education and advice to patients, families, and carers to support long-term well-being.
* Maintain accurate documentation in line with Trust policies and procedures.

Requirements

* HCPC registration as an Occupational Therapist.
* Experience in a community setting is preferred.
* Strong communication, organizational, and problem-solving skills.
