Job Overview:
We are seeking an experienced Band 6 Occupational Therapist to join our community team, working in Marlow and Thame. This role focuses on elderly care, conducting community visits, and delivering person-centered therapy to enhance patients’ quality of life and independence.

Key Responsibilities:

* Manage a community-based caseload, focusing on elderly care and rehabilitation.
* Conduct home assessments and implement interventions to improve safety and independence.
* Collaborate with multidisciplinary teams to coordinate comprehensive care plans.
* Provide advice and support to patients, families, and carers to achieve therapy goals.
* Document patient progress and maintain accurate records in line with trust policies.

Key Requirements:

* HCPC registration as an Occupational Therapist.
* Proven NHS experience in community and elderly care settings.
* Knowledge of current practices in community rehabilitation and home safety adaptations.
* Strong communication, organizational, and teamwork skills.
* Access to a vehicle and a valid driving license for community visits.
