About The Role

Are you a motivated and compassionate nurse looking for a role where you can make a real difference every day?

We are looking for a full-time Band 6 Hospital Liaison Nurse (35 hours per week) to join the Southend STARS service, working across Southend Hospital and the STARS community service. This is a key clinical role supporting patients admitted with drug or alcohol related needs, ensuring they receive the right support in hospital and a smooth transition back into community treatment.

This is an exciting opportunity to work at the intersection of acute healthcare, substance misuse treatment, and public health, helping to reduce harm and improve long-term outcomes for vulnerable patients.

In this role you will:

* Complete comprehensive clinical assessments and risk evaluations for patients with substance misuse needs
* Support safe alcohol and opioid withdrawal management alongside hospital medical teams
* Lead on Hepatitis C screening, testing and referrals into treatment pathways
* Deliver harm reduction interventions including brief advice, BBV awareness and naloxone provision
* Develop effective discharge plans to ensure continuity of care between hospital and community services
* Refer patients into detoxification, treatment programmes and structured recovery support
* Conduct Fibro Scans and ECGs as part of holistic clinical care

Partnership working is central to this role. You will:

* Act as the key link between hospital teams and community substance misuse services
* Work closely with hepatology and liver teams to support patients into Hepatitis C treatment
* Support frequent hospital attenders to improve engagement and reduce readmissions
* Attend multi-disciplinary meetings to support safeguarding, discharge planning and coordinated care

We are looking for someone who:

* Is a Registered Nurse with relevant Band 6 experience or equivalent
* Has experience working with complex or vulnerable patient groups
* Has strong clinical assessment and care coordination skills
* Is confident working across multidisciplinary teams
* Is passionate about improving outcomes for people affected by substance use
