Job Description:
Acclaim Multi-Specialty Group and JPS Health Network’s Department of Family Medicine are seeking a full-time Advanced Practice Provider to join the High ImpACT Pilot Program in Fort Worth. This innovative program addresses the intersection of mental health and homelessness by integrating high-level, mobile psychiatric and primary care with housing assistance through a Housing First Assertive Community Treatment (ACT) Team.
As the county hospital system, JPS has been serving the residents of Tarrant County for over 100 years. This position offers a unique opportunity to manage complex medical cases while providing compassionate, high-quality care to vulnerable populations in various community settings.
Schedule:
Monday – Friday, flexible hours
Call rotation: 3-4 days at a time, shared among six clinicians
Job Summary:
The Advanced Practice Provider will practice under the supervision of a physician, assisting with all aspects of patient care in outpatient residential settings. Responsibilities include managing chronic and acute medical conditions, conducting diagnostic evaluations, educating patients about preventative care, and building meaningful relationships to support housing stability.
Duties and Essential Job Functions:
Collaborate with supervising physicians to coordinate multidisciplinary care, ensuring optimal patient outcomes.
Conduct thorough reviews of medical charts to assist with diagnosis, treatment, and care planning.
Ensure compliance with JPS Health Network and Acclaim Multi-Specialty Group care guidelines.
Deliver program-specific care, including:
Visiting clients in their preferred settings (e.g., apartments, camps, hotels, streets).
Participating in daily, weekly, and monthly team meetings.
Collaborating with mental health providers to address psychiatric and substance abuse needs.
Transporting clients to medical appointments when necessary.
Perform primary care functions such as:
Managing chronic and acute medical conditions.
Conducting diagnostic tests (e.g., labs, STD screenings, blood sugar levels, vitals, weight).
Refer clients for diagnostic imaging or specialty care (e.g., surgical, women’s health, physical therapy).
Administering immunizations and performing wound care.
Providing disease prevention education, including nutritional counseling and smoking cessation.
Coordinating and participating in support or walking groups for clients with similar health conditions.
Build trust and rapport with clients through relationship-focused visits, understanding that some interactions may not be strictly medical.
Foster a collaborative working environment with providers, clinical staff, and support teams.
Qualifications:
Required Education and Experience:
Master’s Degree in Nursing from an accredited nursing school or Master’s Degree in Physician Assistant Studies.
Minimum of 3 years of independent primary care experience as a Nurse Practitioner or Physician Assistant.
Required Licensure/Certification/Specialized Training:
Current Texas Physician Assistant license or Texas APRN license.
Basic Life Support (BLS) certification.
Advanced Cardiac Life Support (ACLS) certification.
Preferred Licensure/Certification/Specialized Training:
Experience in behavioral health and/or addiction treatment
