Job Overview
We are seeking a dedicated and compassionate Band 6 Community Occupational Therapist to join the team at Midlands Partnership NHS Trust. This role focuses on supporting adults in their own homes who have physical disabilities and long-term health conditions. The main aim of the role is to enable individuals to maintain independence in the community and enhance their quality of life through tailored Occupational Therapy interventions.
Key Responsibilities
* Provide assessments and interventions for adults with physical disabilities and long-term health conditions in a community setting.
* Assist clients in achieving as much independence as possible through rehabilitation and adaptive strategies.
* Work collaboratively with the multidisciplinary team to design and implement care plans.
* Offer advice and support to families and carers to ensure the continuation of therapy goals at home.
* Monitor client progress and adjust treatment plans as necessary to ensure optimal outcomes.
* Promote and enable the client’s autonomy, ensuring they remain as independent as possible within their homes.
Requirements
* HCPC-registered Occupational Therapist.
* Experience in a social care Occupational Therapy role or a community-based setting is highly beneficial.
* Ability to manage a caseload independently and effectively.
* Strong communication and organizational skills.
* A commitment to delivering high-quality, person-centered care.
