About the Role:

As a Social Worker in the Preparing for Adulthood, Learning Disability and Autism Team, you will:

* Assess individual needs and develop personalised support plans.
* Coordinate with families, caregivers, and other professionals.
* Advocate for clients to access necessary services and resources.
* Provide ongoing support and monitor progress towards independence.

Requirements:

* Qualified and registered social worker.
* Passionate about making a difference in the lives of adults.
* Eager to work in a supportive and progressive environment.

Requirements:

* Degree in Social Care
* Experience in adult’s safeguarding
* Social Work England Registration
